If you're an NASD Registered Representative or other government-regulated person or entity enjoined or prohibited by any association or professional rules from collecting undisclosed commissions, you MUST select the rebate option.

First, read the 401(k) Easy Referral Partner Agreement.
All Referral Partner applicants must agree to the terms of the Agreement for their Referral Partner Applications to be acceptable.

Second, complete and submit your 401(k) Easy Referral Partner Application (below).
You'll receive an e-mail confirmation (or other, if you don't have e-mail) that you have been added to our Referral Partner Program and are eligible for the rights and benefits thereof as soon as your completed application has been processed. Once you receive this written confirmation, you're eligible to earn referral commissions (or have customers you refer be eligible for the $200 rebate).

Any change to your Application will not become effective, however, until we notify you of its acceptance in writing.
